- Special Circumstances Require Additional Forms: If your condition now qualifies for Aid and Attendance, Individual Unemployability, or needs home or auto adaptation, submit the relevant additional forms:

- Individual Unemployability: VA Form 21-8940 and VA Form 21-4192.
- Specially Adapted Housing: VA Form 26-4555.
- Auto Allowance: VA Form 21-4502.
